A valid copy of the practice certificate of the person in charge of professional and technical expertise of the medical examination and treatment establishment; the person in charge of the specialized section of the medical examination and treatment establishment.

Photos of the facility's signboard and photos of the facility's specialized departments.

An explanation of professional equipment and technical facilities.

A written commitment to comply with the provisions of the law on prices; price list for medical examination and treatment and other services of the facility.
2. Procedure for applying for a polyclinic operating license
Step 1: The medical examination and treatment facility sends an application for an operation license to the Department of Health. Records include:

An application for an operation license is made according to the form specified in Appendix 13 issued with this Circular.

A certified true copy of the establishment decision for state medical examination and treatment establishments or the business registration certificate for private medical examination and treatment establishments or the investment certificate, for medical examination and treatment with foreign investment.

Certified copies of practice certificates of all practitioners and a list of registered practitioners of the medical examination and treatment facility according to the form.

A declaration of physical facilities and medical equipment of the medical examination and treatment establishment using the form specified in Appendix 14 to this Circular.

Personnel records of the person who works in the medical profession at the facility but is not required to issue a practicing certificate.

Documents proving that the medical examination and treatment establishment meets the conditions on physical foundations, medical equipment, and personnel organization suitable to the scope of professional activities.

Estimated scope of professional activities: The medical examination and treatment facility proposes the scope of professional activities, the list of techniques to be performed on the basis of the list of professional techniques promulgated by the Minister of Health.
Step 2: After receiving the application for an operation license, the provincial Department of Health shall send it to the medical examination and treatment facility requesting the operation license.
Step 3: Within 90 days from the date on which the application is received, if there is no additional request, the application-receiving agency must consider and issue an operation license; if the operation license is not granted, re-granted or adjusted, a written reply clearly stating the reason must be given. In case the application is not valid, within 10 working days from the date on which the application is received, the application-receiving agency must send a written notice to the medical examination and treatment establishment requesting the issuance of the application operating license to complete the profile. The written notice must specify which documents are supplemented and which contents need to be amended.

From 65 days to 70 days from the date of receiving complete and valid dossiers.
